Job summary

Impact Health PC in Longmeadow, MA seeks a Nurse Practitioner to deliver comprehensive medical care in skilled nursing facilities. You will assess, manage acute and chronic conditions, and create individualized treatment plans while collaborating with physicians, staff, and interdisciplinary care teams.

You will have autonomy within your scope of practice, strong clinical support, and competitive earning potential, with a Monday–Friday schedule, no nights or weekends, and comprehensive benefits

Qualifications

• Active NP license in Massachusetts.
• 2+ years post-certification experience preferred.
• Experience in skilled nursing facilities or primary care preferred.
• Ability to manage acute and chronic conditions in older adults.

Responsibilities

• Provide comprehensive Medical/Primary Care to residents in SNFs.
• Assess, diagnose, and manage acute and chronic conditions.
• Develop individualized, evidence-based treatment plans.
• Order and review medications, laboratory tests, imaging, and other diagnostic studies.
• Collaborate with physicians, nursing staff, specialists, and interdisciplinary care teams.
• Educate patients and families regarding medical conditions and treatment plans.
