Impact Health PC in Monroe, CT is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to deliver medical/primary care to residents of assisted living facilities, managing acute and chronic conditions and coordinating with the care team.

Qualifications

• Active NP/PA license in Connecticut
• Minimum 2 years of post-certification experience preferred
• Primary care, assisted living, geriatric, or similar clinical experience preferred
• Strong clinical assessment, communication, and organizational skills

Responsibilities

• Provide comprehensive Primary Care to residents in Assisted Living Facilities.
• Assess, diagnose, and manage acute and chronic medical conditions.
• Develop evidence-based, individualized treatment plans and coordinate care with staff, physicians and families.
• Order and review medications, labs, imaging, and other tests as appropriate.
• Complete timely clinical documentation and place appropriate orders.
• Educate patients and families regarding conditions and treatment plans.
