Impact Health PC in North Haven, CT is hiring an NP to deliver comprehensive Medical/Primary Care to residents in skilled nursing facilities. You will assess and manage acute and chronic conditions, develop individualized plans, and follow up with patients while collaborating with physicians and the care team.

Qualifications

• Active Nurse Practitioner (NP) license in CT.
• Minimum 2 years of post-certification clinical experience preferred.
• Experience in skilled nursing facilities, primary care, hospitalist medicine, ED, or ICU preferred.
• Strong clinical assessment, communication, and organizational skills.
• Comfortable managing acute and chronic medical conditions in older adults.
• Ability to work independently within scope of practice and with an interdisciplinary care team.
• Proficiency with EHRs and clinical technology.

Responsibilities

• Provide comprehensive Medical/Primary Care to residents in SNFs.
• Assess, diagnose, and manage acute/chronic conditions.
• Perform routine evaluations and follow-up care.
• Develop individualized treatment plans with evidence-based approaches.
• Order and review medications, labs, imaging, and diagnostics.
• Document accurately and place orders.
• Collaborate with physicians, staff, and specialists.
• Educate patients and families about conditions and plans.
