Job summary

Johns Hopkins Community Physicians seeks a Nurse Practitioner to join our Home-Based Primary Care Program in Baltimore, Howard County, and Anne Arundel County in Maryland. The NP will evaluate and treat home-bound, medically complex older adults, coordinating care with the Medical Director and other team members.

Three to five years NP inpatient or outpatient practice preferred; Maryland licensure and CPR certification required.

Qualifications

• Must have Maryland NP licensure and CPR certification.
• 3 years’ experience in primary care or acute/urgent care preferred.
• Palliative or geriatric certification/experience preferred.
• Must have a working vehicle to travel to patient homes.

Responsibilities

• Provide medical care for home bound and medically complex older adults.
• Evaluate and treat patients receiving primary care at home.
• Coordinate care with team members and specialists.
• Participate in weekly inter-disciplinary team meetings.
