Job summary

Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members in and around Aliquippa, PA. This role offers flexible scheduling and the ability to work outside a traditional clinic.

Responsibilities include traveling to patients' homes, conducting non-invasive physical assessments, reviewing medical histories, and documenting findings. NPs will provide basic wellness education and coordinate with scheduling.

Qualifications

• Master’s Degree in NP or commensurate experience with NP license.
• Active State NP license and national board certification are required.
• Current BLS certification is required.
• 12 months of paid advanced clinical provider experience preferred but not required.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic patient education on wellness and preventative care.
