Job summary

Signify Health in the United States is seeking a Clinician Field Mentor to mentor clinicians, perform In-Home Evaluations, and drive high-quality care. The role balances 80% field visits with 20% peer mentorship, supporting new hires and active clinicians while collaborating with Clinical Leaders and Operations Managers.

Ideal candidates have a clinical background with licensure, strong mentoring skills, and experience with Signify IHE tools and processes, plus flexibility to travel regionally.

Qualifications

• Clinical background and State Licensure (e.g., MD/DO, NP/PA)
• Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds
• Multi-state licensure (preferred)
• 5–7 years of clinical experience (preferred)
• 6 months performing Signify Health in-home health evaluations (preferred)

Responsibilities

• Complete In-home Visits (IHEs, Focused Visits, Video Visits) with KPI compliance
• Test new Diagnostic & Preventive Services and provide feedback
• Provide 40 hours weekly and 128+ capacity units per month with coaching day
• Maintain visit completion rate of 88+ visits per month
• Provide dedicated 90-day mentorship to new hires
• Offer group support and real-time field assistance
• Escalate clinical/operational issues to Clinical Leaders as needed
• Attend regular meetings and participate in events
