Job summary

Uplift HomeCare LLC is hiring a Certified Occupational Therapy Assistant (COTA) for PRN home health in Winter Haven, FL and surrounding areas. You will deliver OT interventions under supervision, follow treatment plans, and help patients improve daily living activities in the home.

The role emphasizes flexibility, independence, and meaningful patient care within a supportive clinical team. Requires FL COTA license, NBCOT, and AHCA background screening, plus at least 1 year in home health.

Qualifications

• Active Florida COTA license.
• Graduation from an accredited COTA program.
• Current NBCOT certification, as applicable.
• At least one year of home health experience.
• AHCA Level II background screening completed.
• Strong communication, organizational, and documentation skills.
• Valid driver's license with reliable transportation.
• Ability to work independently under OT supervision.

Responsibilities

• Provide occupational therapy interventions under the supervision of a licensed OT.
• Implement individualized treatment plans and physician orders.
• Assist patients with activities of daily living and functional mobility.

What You'll Do

• Provide occupational therapy interventions under the direction and supervision of a licensed Occupational Therapist.
• Implement individualized treatment plans and applicable physician orders.
• Help patients improve activities of daily living, functional mobility, safety, and independence.
• Educate patients and caregivers on therapeutic activities, adaptive strategies, and home safety.
• Monitor patient responses and report progress or changes in condition to the supervising OT.
• Complete accurate, timely, and compliant clinical documentation.
• Communicate effectively with the clinical team to support positive patient outcomes.

What We're Looking For

• Active Florida Certified Occupational Therapy Assistant (COTA) license.
• Graduation from an accredited Occupational Therapy Assistant program.
• Current NBCOT certification, as applicable to the position and licensing requirements.
• At least one year of home health experience required.
• Required AHCA Level II background screening.
• Strong communication, organizational, and documentation skills.
• Valid driver's license, current auto insurance, and reliable transportation.
• Ability to work independently while following the supervising Occupational Therapist's direction.
