Job summary

Uplift HomeCare, LLC in Tampa and Hillsborough County, Florida, is seeking a Certified Occupational Therapy Assistant to provide one-on-one home health services. You’ll work under the supervision of a licensed OT to help patients maintain independence in their homes.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Active Florida COTA license.
• NBCOT certification (as applicable to position).
• Minimum of one year of home health experience.
• AHCA Level II background screening.
• Strong communication, organization, and documentation skills.
• Ability to work independently with clinical team.

Responsibilities

• Provide occupational therapy interventions under supervision of a licensed OT.
• Implement individualized treatment plans and therapeutic activities.
• Educate patients and caregivers on exercises, safety and fall prevention.
• Monitor patient responses and document progress in chart.
• Coordinate care with the supervising OT and health team.
• Promote safety and independence in the home setting.

What You’ll Do

As a Home Health COTA, you’ll provide occupational therapy interventions under the supervision and direction of a licensed Occupational Therapist.

Responsibilities include:

• Implement individualized occupational therapy treatment plans.
• Provide therapeutic activities and exercises to improve daily living skills, coordination, strength, mobility, and functional independence.
• Help patients work toward goals established by the supervising OT.
• Educate patients and caregivers on prescribed exercises, adaptive techniques, home safety, and fall prevention.
• Monitor patient responses to treatment and report changes or concerns to the supervising OT and appropriate clinical personnel.
• Coordinate care with the supervising OT and other members of the healthcare team.
• Complete accurate, timely, and compliant clinical documentation.
• Promote patient safety, independence, and quality of life in the home environment.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Active Florida Certified Occupational Therapy Assistant (COTA) license.
• Current NBCOT certification, as applicable to the position and licensure requirements.
• Minimum of one year of home health experience required.
• AHCA Level II background screening.
• Strong communication, organization, and documentation skills.
• Ability to work independently while collaborating effectively with the clinical team.
• Valid driver’s license, reliable transportation, and current auto insurance.

$250 Sign-On Bonus

Eligible new hires may qualify for a $250 sign-on bonus after meeting all applicable eligibility requirements:

• Successfully complete required onboarding.
• Remain actively employed with Uplift HomeCare for 90 days.
• Maintain good standing and meet company requirements, including timely completion of required documentation.

The bonus is paid through regular payroll following completion of the 90-day eligibility period and is subject to applicable taxes and company eligibility requirements.
