Qualifications

• Active Texas RN license
• CPR certification preferred
• 2 years hospice experience preferred
• Excellent communication and assessment skills
• English language proficiency
• Reliable transportation and valid driver's license

Responsibilities

• Coordinate and oversee patient care for hospice patients and families.
• Collaborate with the interdisciplinary team to plan and implement care.
• Educate patients and families about care plans and end-of-life options.
• Document clinical notes and care activities clearly and timely.
