Job summary

A leading healthcare service provider is seeking a remote support specialist to assist with patient care transitions and manage documentation. The role requires strong communication skills and the ability to work independently. Ideal candidates will have experience with Epic EHR and a high school diploma.

Qualifications

  • Knowledge and experience with Epic EHR.
  • Willingness to work rotating weekends.

Responsibilities

  • Assist with UR transmission and manage a high volume of faxes.
  • Coordinate and implement the Transition of Care plan for patients.
  • Maintain accurate documentation in electronic medical records.

Job description

Holidays : Required

Contract Duration : 3-6 months

Location : 100% remote - Supporting Pacific Time Zone. Preference for candidates in NEV / OR. No WA candidates.

Requirements :

  • Knowledge and experience with Epic EHR
  • Willingness to work rotating weekends

Responsibilities :

  • Assist with UR transmission, manage a high volume of faxes and voicemails, and perform data entry
  • Coordinate and implement the Transition of Care (TOC) / Discharge (DC) plan for ambulatory patients
  • Collaborate with Case Managers and Social Workers to assess patient needs and develop TOC plans
  • Ensure timely discharge by monitoring progress and addressing barriers
  • Facilitate post-discharge referrals and authorizations
  • Maintain accurate documentation and communication in electronic medical records
  • Provide clerical support including copying, faxing, scanning, and data entry
  • Participate in daily team huddles and department meetings
  • Contribute to team decision-making and problem-solving
  • Build and maintain positive relationships with customers and partners

  • Demonstrate tact and respect for all customers
  • Resolve issues using effective communication skills
  • Comply with confidentiality policies and HIPAA regulations

Education : HS Diploma or equivalent education / experience

Skills and Knowledge :

  • Strong oral and written communication skills
  • Effective interpersonal and time management skills
  • Ability to work independently with minimal direction
  • Intermediate PC and word processing skills
