Jefferson Health Plans seeks a professional nurse to join the ambulatory clinics, applying the nursing process to address patients' physical, psychological, emotional, and social needs while collaborating with primary care providers. Must hold a current nursing license in Pennsylvania.

The role emphasizes evidence-based practice, care coordination, patient education, and safe, culturally competent care within an inter-professional team. Strong communication and documentation in EMR are essential.

Formación

• Graduate of an accredited school of nursing required.
• Knowledge of patient- and family-centered practice and respect for diverse backgrounds.
• Effective communication and partnership with patients and families.
• HIPAA compliance and confidentiality maintenance.
• Ability to document and coordinate care within EMR per standards.
• BLS certification required; Pennsylvania RN licensure.

Responsabilidades

• Provide nursing care in ambulatory clinic settings using the nursing process.
