Job summary

Qualifications

• Registered Nurse currently licensed to practice nursing in the state of North Carolina.
• Minimum of one-year experience as a professional nurse in an acute care setting.
• Current CPR certification (BCLS).
• Previous experience working with terminally ill patients preferred.
• Basic computer skills and experience with electronic medical records.
• Ability to communicate well with patients, families, staff, physicians and the public.
• Possess a valid Driver's License and reliable transportation.

Responsibilities

• Admit patients to the agency by collecting medical history and performing a physical assessment; evaluate home circumstances and teaching needs.
• Develop and implement the patient care plan in collaboration with other Agency professionals.
• Manage assigned cases and provide skilled nursing care per physician's plan and agency policies.
• Educate and counsel patients and families on nursing care needs, disease process, and end-of-life care.
• Supervise and coordinate with Home Health Aide and other staff; document changes and report to supervisors.
