Hospice of the Piedmont is seeking a Registered Nurse for our Night/Evening/Weekend Services team serving Charlottesville and surrounding areas. This role emphasizes autonomous bedside nursing, triage by phone, and coordination with the medical team and vendors during nights and weekends.

Responsibilities include triage, home visits, patient assessments, and maintaining documentation while delivering compassionate care and maintaining patient confidentiality.

Qualifications

• Two years of relevant nursing experience.
• Must hold a Virginia RN license.
• Driver’s license and reliable transportation required.

Responsibilities

• Triage and provide nursing care to patients in home settings.
• Make home visits to assess patient needs and provide care.
• Perform nursing evaluations for new admissions and determine eligibility.
• Communicate changes to the medical team and document promptly.
