Job summary

OhioHealth Corporation is seeking a Transfer Center Nurse to triage and coordinate patient transfers across eight facilities. The role requires careful clinical judgment, strong communication, and multitasking to ensure safe, timely admissions and bed placement.

Responsibilities include coordinating inbound transfers, arranging transport, and implementing multiple care pathways. A 36-hour weekly night shift is typical in this 24/7 center.

Qualifications

• Bachelor's Degree in Nursing required.
• RN license with Ohio Board of Nursing.
• Excellent verbal and written communication and organizational skills.
• Computer skills (CareConnect).
• Typing skills.
• Experience: 3–5 years in emergency department/critical care.

Responsibilities

• Maintains flow of patients throughout the hospital, including ED and nursing units.
• Assesses admission for appropriate level of care and resources.
• Triages and reviews documentation for direct admissions/ED transfers.
• Initiates clinical pathways for emergencies and admissions.
• Screens admissions for appropriateness and directs to optimal care.
• Contacts physicians for clarification and changes in care needs.
• Plans and coordinates safe transfers of inpatient patients while coordinating bed management.
• Collaborates with nursing administration to allocate resources.
• Evaluates patient status to ensure appropriate placement and care level.
• Provides superior customer service to patients, families, and other stakeholders.
• Provides bed management education to interns, residents, and physicians.
• Identifies opportunities for process improvement and liaison with Central Ohio hospitals.
