Job summary

CBH Care seeks a full-time night-shift supervisor/case manager in Rutherford, NJ, 12:30am–8:30am. You will supervise, manage cases, aid daily living activities, help with budgeting, link to benefits, and educate on medications.

A relevant degree and a valid driver's license are required; prior experience is preferred. We offer competitive pay, excellent health benefits, a 403(b) plan with employer contributions after one year, generous PTO, 10 holidays plus discretionary days, and opportunities

Qualifications

BSW in social work, BA/BS in psychology, criminal justice, sociology, or a related field
BA in an unrelated field with two years of previous experience
AA in a related field with two years of previous experience
Previous experience desired
Valid Driver's License

Responsibilities

Provides continuous supervision and case management services to assigned consumers on caseload
Assists consumers with their activities of daily living skills, budgeting, and linkage to applicable benefits
Provides consumers with medication education and observes their self-administration of medication
Must adhere to the agency's driving policy of transporting consumers in a safe and responsible manner
Provides input to treatment and service plans as well as their implementation
Must be able to document all relevant information to the consumers' electronic chart records
