Job summary

CBH Care in Rutherford, NJ is seeking a dedicated care coordinator to provide supervision and case management for a caseload of consumers, delivering supports aligned with individual treatment plans. The role helps clients with daily living skills, budgeting, and connecting them to benefits, and includes medication education and safe self-administration oversight.

The position is Monday–Friday, 3:00pm–11:00pm, full-time, with competitive benefits and growth opportunities within a mission-driven

Qualifications

• BSW in social work is required.
• BA/BS in psychology, criminal justice, sociology, or a related field.
• BA/BS in psychology, criminal justice, or sociology is acceptable as part of the degree mix.
• AA in a related field with two years of experience is acceptable.
• Two years of previous experience is preferred.
• Valid Driver’s License is required.

Responsibilities

• Provides continuous supervision and case management services to assigned consumers on caseload.
• Assists consumers with activities of daily living skills, budgeting, and linkage to applicable benefits.
• Provides consumers with medication education and observes their self-administration of medication.
• Must adhere to the agency’s driving policy of transporting consumers in a safe and responsible manner.
• Provides input to treatment and service plans as well as their implementation.
• Must be able to document all relevant information to the consumers' electronic chart records.
