Job summary

Qualifications

• Must possess a current, valid RN license in state of practice.
• BLS certification is required.
• Experience in the clinical area for which applying is desired.

Responsibilities

• Assess patient physical, psychological, social, spiritual, educational and discharge planning needs.
• Formulate a plan of care with patient and family input.
• Implement the plan of care and supervise care delivery.
• Evaluate patient progress and revise care plan as needed.
• Document assessment, planning and evaluation in the patient record.
• Promote evidence-based care and interprofessional collaboration.
• Provide leadership and mentor staff when appropriate.
