Job summary

Qualifications
- BSN from an accredited school of nursing; if RN has an ADN, must complete BSN within 5 years of start date.
- Minimum 1 year of Registered Nurse clinical experience.
- BLS certification from American Heart Association.
Responsibilities
- Perform comprehensive patient health assessments and interpret data sets.
- Formulate individualized plans of care based on assessments.
- Deliver patient care in a patient/family centered manner and coordinate transitions.
- Advocate for patients' rights and provide counseling as a resource.
- Document all patient care activities and support team documentation.
- Evaluate outcomes and contribute to performance improvement initiatives.
- Collaborate with the care team and provide clinical guidance as needed.
