Qualifications

• BSN from an accredited school; ADN acceptable with BSN within 5 years of start date.
• Minimum 1 year of Registered Nurse clinical experience.
• BLS certification from American Heart Association.
• Licensed or eligible for Virginia RN licensure (or multi-state license).

Responsibilities

• Performs comprehensive patient health assessments and interprets data sets.
• Formulates individualized plans of care based on assessments.
• Provides patient/family centered care and coordinates transitions.
• Documents all patient care activities and supports care team documentation.
• Evaluates outcomes and participates in performance improvement initiatives.
• Mentors staff and acts as a clinical resource as needed.
