Job summary

Topaz Fiscal Services is seeking an LVN/LPN to organize and direct the nursing care for residents on assigned shifts. You will ensure compliance with regulations, document progress, and coordinate with therapists to meet 24-hour rehabilitation needs.

The role requires a current LVN/LPN license and at least two years of related experience, with good standing in the state registry. Strong assessment, documentation, and emergency response skills are essential.

Qualifications

• Must be able to provide quality nursing care to residents.
• Proficient in taking and interpreting vital signs and documenting findings.
• Experience administering prescribed medications and following nursing techniques.
• Ability to transcribe orders and maintain accurate progress notes.
• Respond appropriately to emergencies in compliance with policies.

Responsibilities

• Oversee total nursing care for residents during shift.
• Explain procedures, treatments, and care plans to residents.
• Administer medications and treatments per approved techniques.
• Transcribe physician orders and document care delivered.
• Conduct frequent rounds to evaluate care delivery and resident response.
• Maintain safety and comfort of residents and respond to changes in condition.
• Coordinate rehabilitation needs with therapists to ensure 24-hour care.
• Document nutrition orders and liaise with Dietary Department.
• Supervise CNAs and ensure care plans are followed and updated.
• Support care conferences and participate in quality improvement activities.
