Job summary

ProMedica’s Eidi Family Rehabilitation Center in Sylvania, OH seeks a Registered Nurse to join a patient‑centered care team. The RN coordinates a patient’s plan of care with families and the health care team, ensuring sensitive, respectful care that supports diverse patient needs.

You will assess, diagnose, plan, implement and evaluate the care plan, working with nurses and support staff to improve patient outcomes. This is a per diem, night‑shift role in a mission‑driven health system.

Qualifications

• Current State license as a Registered Nurse.
• Current CPR training.
• The RN in Ohio must create twenty‑four contact hours of continuing education during licensure period. At least 1 hour must relate to Ohio nurse practice code and rules.

Responsibilities

• Coordinate patient’s plan of care with patients, families and the health care team.
• Assess, diagnose, plan, implement and evaluate nursing care.
• Collaborate with fellow nurses and support staff to reach patient care goals and improve patient outcomes.
