Qualifications

• Graduate of an accredited nursing program.
• Minimum of 3+ years RN experience in Emergency, Acute Care, or similar fast-paced environment.
• Experience with clinical data collection, ethical principles, and patient advocacy.

Responsibilities

• Coordinate care and guide patients through health journeys without bedside care.
• Conduct telephonic patient assessments and remote intake using evidence-based protocols.
• Provide health education and support healthier behaviors across the care continuum.
• Utilize telemonitoring tools and technology to track progress and intervene when needed.
• Collaborate with team care planning, mentor colleagues, and champion new care delivery approaches.
• Represent GMR’s programs within the community and educate patients about available resources.
