Job summary

Green Acres is hiring for a dedicated LPN/RN to join our night shift nursing team. You will conduct patient assessments, coordinate care with the healthcare team, and provide bedside care in a long-term care setting.

Qualifications

• Current LPN/RN license in the state.
• At least one year of LPN or RN experience in a long-term care setting is preferred.

Responsibilities

• Consults and coordinates with healthcare team members to assess, plan, implement and evaluate patient care plans.
• Prepares and administers prescribed medications and records them.
• Records patients' medical information and vital signs.
• Monitors and adjusts equipment used on patients and interprets displays.
• Initiates corrective action when patient shows adverse symptoms.
• Provides bedside care for a variety of medical patients.
• Changes dressings, inserts catheters, starts IVs.
• Assists physician during examination and treatment.
• Participates in discharge planning and documents care timely.
