Job summary
Green Acres Healthcare in Mayfield, KY is hiring for a Full-time Night Shift LPN/RN to provide compassionate patient care in a long-term facility. You will consult with the healthcare team, administer medications, monitor patient status, and support discharge planning.

Qualifications
- Current LPN/RN license in the state.
- At least 1 year of LPN/RN experience in long-term care preferred.
- Ability to work full-time night shifts and collaborate with a healthcare team.
Responsibilities
- Coordinate and assess patient care plans with the healthcare team.
- Prepare and administer medications as prescribed and document accordingly.
- Record patient medical information and vital signs.
- Monitor equipment and interpret electronic displays.
- Initiate corrective action for adverse symptoms.
- Provide bedside care for a variety of medical patients.
- Change dressings, insert catheters, start IVs.
- Assist physician during examinations and treatments.
- Respond to life-saving situations per nursing standards.
- Participate in discharge planning.
- Utilize time management skills.
- Document care information accurately and promptly.
- Perform other duties as assigned.
