Job summary

SnapCare is recruiting an RN for a 13-week night shift contract in West Allis, WI, focused on critical illness and rehab. The nurse will perform assessments soon after admission, develop care plans, and participate in interdisciplinary rounds.

Responsibilities include documentation, discharge planning, team communication, and supervision of LPNs/CNAs. A strong bedside background and current RN license with BLS/ACLS are required.

Qualifications

• Minimum two years of bedside clinical experience.
• BLS and ACLS certifications required prior to first shift.
• RN license required to practice.

Responsibilities

• Perform nursing assessments within 8 hours of admission.
• Develop and implement individualized care plans based on patient data.
• Document assessments, interventions, and patient responses in the medical record.
• Participate in rehabilitation programs on the unit and use the nursing process.
• Initiate discharge planning from day of admission and begin patient teaching.
• Communicate changes in patient status to the attending physician and Charge Nurse.
• Provide direction, training, and supervision of LPNs, CNAs, and orientees.
• Assume charge responsibility after orientation as requested.
• Maintain standards of personal appearance, attendance and punctuality.
