Qualifications

• BLS Provider required within 1 month of hire or transfer date.
• RN license required prior to hire date or transfer date.
• Diploma from an accredited nursing program required or equivalent licensure.

Responsibilities

• Implements and monitors patient care plans; documents and communicates patient status.
• Coordinates multidisciplinary care as primary liaison for timely, well-coordinated treatment.
• Notes and carries out physician and nursing orders as directed.
• Assesses and coordinates discharge planning needs with the healthcare team.
