Qualifications

• Associate's of Nursing required.
• Basic Life Support - CPR certification needed.
• Registered Nurse (RN) accreditation necessary.

Responsibilities

• Assess, develop, implement, and evaluate care plans.
• Conduct thorough patient assessments.
• Liaise with post-hospital care and community resources.
• Manage patient transitions upon discharge.
• Participate in practice group meetings and peer reviews.
