Job summary

Responsibilities

• Collect comprehensive data pertinent to the patient's health or situation.
• Perform age and disease-specific assessment/data collection independently.
• Develop and implement a plan of care with coordinated interventions.
• Communicate and coordinate with the healthcare team and patient/family.
• Document patient care at the point of care in the electronic system.
• Evaluate progress toward attainment of outcomes.
