RWJBarnabas Health seeks a PD Transfer Center Specialist to support patient transfers from the Transfer Center. You will handle referrals, verify critical information, and coordinate bed placement and transport, ensuring smooth patient transitions into RWJBH hospitals.

Qualifications

• High school diploma or GED required.
• Experience in healthcare office or hospital operations preferred.
• Proficiency with Microsoft Office and typing speed of 40 wpm.

Responsibilities

• Serve as the point of contact for referrals and initiate transfer processes.
• Verify patient information, coordinate bed availability, and ensure timely transfers.
• Enter referrals into Epic and coordinate with Patient Access and receiving facilities.
• Document data accurately, protect PHI, and maintain compliance with HIPAA and policies.
• Answer calls, follow up with clinicians, and support efficient transfer workflows.
• Coordinate transports and communicate status with departments and facilities.
