Qualifications

Responsibilities

• Coordinate referrals from interdisciplinary team members and facilitate care management.
• Ensure quality patient care and regulatory compliance.
• Participate in daily rounds to review progression and discharge plans.
• Remain at the bedside with assigned patients unless relieved.
• Assist in calming and reorienting patients during agitation.
• Record observations accurately and promptly.
• Support patients with complex psychosocial needs under mentorship.
• Perform other duties as assigned.
