Job summary

MercyOne Health Ministries, part of Trinity Health, seeks a Patient Logistics Access Specialist in Des Moines to be the initial contact for transfers and admissions. You will gather demographics, referrals, and essential data to start transfer workflows for timely clinical review.

You will route calls to the correct clinician or operations partner, maintain EMTALA-sensitive documentation, and support capacity navigation across facilities, EMS, and transport teams.

Qualifications

• Health care experience required.
• High school diploma or GED required.
• Associate’s Degree or equal experience preferred.
• Must be comfortable working in a collaborative, shared leadership environment.

Responsibilities

• Knows, understands, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors and decisions.
• Serves as the first point of contact for incoming transfer, admission, and consultation requests; ensures calls are answered promptly and routed correctly.
• Collects and verifies accurate demographic, referral, and operational information to initiate the transfer workflow.
• Manages multi-line phone activity, prioritizing calls and minimizing delays for safe patient transitions.
• Documents all call details in electronic health records and transfer management systems with accuracy and regulatory awareness.
• Routes calls and information to the appropriate clinician or operational team based on established protocols.
• Supports EMTALA-sensitive processes by ensuring complete intake documentation and escalating red flags.
• Coordinates non-clinical transport logistics, including contacting EMS, confirming availability, and monitoring ETA updates.
