• Assesses and evaluates care for an assigned patient group using evidence-based practice.
• Collaborates with patient and family to develop and implement a care plan.
• Communicates pertinent information to the care team in a timely manner.
• Keeps patient and family apprised of updates.
• Documents assessments, interventions and outcomes accurately.
