Responsibilities

Delivers safe and appropriate care to patients, following nursing skills inventory and patient needs.
Coordinates nursing care planning with an interdisciplinary team and makes clinical decisions.
Collaborates with healthcare team to achieve positive outcomes based on data and standards.
Coordinates discharge and transfer plans for patients.
Participates in program evaluation, process improvement, and outcomes assessment.
Adheres to regulatory standards and Memorial Hermann policies and ensures resource stewardship.
Supports professional growth; may serve as preceptor or mentor.
