Job summary

The Crossings at Riverchase in Birmingham, AL, is seeking a Caregiver for night shifts in Memory Care to support residents with daily living activities and specialized memory care needs.

Qualifications

• High school diploma or equivalent (GED) required.
• Nursing Assistant Training Program certificate (approved by the State Board of Nursing) if required.
• CPR and First Aid certification if required.
• Must be able to sit, stand, walk, climb stairs, reach, bend, stoop, crouch and kneel; lift up to 50 pounds unassisted; assist with transfers, repositioning and mobility.

Responsibilities

• Follow individualized plan of care (ISP) for each resident; observe changes and report to nursing supervisor; document daily care and red flags during night shifts.
• May check and record vital signs, weights, and other duties as assigned per state regulations and policies.
• Assist residents with activities of daily living (bathing, oral care, toileting, dressing) and perform restorative nursing techniques (positioning, ADL training,Range of Motion).
• Encourage participation in activities; assist and transport residents to activities within the facility.
• Assist with transfers and ambulation using proper body mechanics and equipment; provide transportation to appointments.
• Respond to urgent calls promptly and perform other duties as assigned.
