Job summary

Bridge Senior Living LLC is hiring a Wellness Nurse for the part-time night shift (7p-7a) to deliver coordinated care in collaboration with physicians and care teams. You’ll serve seniors with compassion and help create a positive home-like environment residents are proud to call home.

The role requires a valid LPN license, CPR, and at least 1 year of related experience, with strong communication to residents, families, and the care team. Sign-on bonus is included.

Qualifications

• High school diploma or GED required.
• LPN License in applicable state.
• First Aid & CPR certification required.
• At least 1 year of related work experience.

Responsibilities

• Assessment, planning, implementation, and evaluation of our residents.
• Provide high level of care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing our residents’ overall condition and behavior.
• Assist the Director of Wellness in the development and update of resident care plans.
• Maintain ongoing communication with residents, families, and physicians regarding the resident’s care.
