Job summary

The Aspenwood Company - The Crestmoor at Green Hills seeks a compassionate Wellness Nurse to administer medications, oversee resident care, and support a team focused on dignity, safety, and person-centered service in a senior living community.

This full-time night shift role requires LVN/LPN licensure, BLS, and 6 months to 1 year of long-term care experience; you will coordinate staff, document care, and communicate with physicians and families. A $2,500 sign-on bonus is offered with benefits.

Qualifications

• Current LVN/LPN licensed in state of employment.
• BLS/CPR certification required.
• 6 months to 1 year experience in Assisted Living or Long-Term Care.
• Proficient in EHR and basic computer skills.
• Strong verbal and written communication skills.
• Demonstrated critical thinking and problem-solving abilities.
• Knowledge of HIPAA regulations and compliance standards.
• Ability to respond empathetically to dementia-related behaviors.

Responsibilities

• Administer medications safely per physician orders and regulations.
• Oversee medication aides and conduct monthly reconciliation and audits.
• Coordinate daily staff assignments to ensure consistent resident care.
• Communicate effectively with physicians, families, and care teams.
• Provide direct care including bathing, dressing, grooming, mobility, and incontinence support.
• Respond to medical and behavioral emergencies with sound clinical judgment.
• Complete clinical documentation including assessments and service plans.
• Document incidents and communicate with leadership promptly.
• Monitor care team documentation for accuracy and completeness.
• Support orientation and ongoing training of care staff.
• Assist with scheduling in collaboration with leadership.
• Attend required in-services and continuing education.
