Job summary

BrightStar Care – Cleveland is seeking a skilled LPN with Trach and Vent experience to provide high-quality nursing care in home and facility-based settings in Elyria and surrounding areas. The role emphasizes airway management, critical care, and compassionate, one-on-one patient care for pediatric and memory/dementia patients.

Location: Elyria, Lorain. Schedule includes nights with a Monday–Sunday 8AM–4PM rotation.

Qualifications

• Experience in Trach and Vent; Pediatric care.
• Strong clinical judgment and independent decision-making skills.
• Ability to lift up to 50 lbs and assist with patient mobility.

Responsibilities

• Provide skilled nursing care for pediatric, memory/dementia, and complex care patients.
• Perform and manage Trach & Vent care, including airway and ventilator support.
• Monitor patient condition and respond to clinical changes appropriately.
• Assist with ADLs and patient mobility as needed.
• Implement infection control and patient safety protocols.
• Administer care per physician orders and clinical guidelines.
• Maintain accurate, timely clinical documentation.
• Communicate effectively with care team, patients, and families.
• Ensure compliance with all safety, hygiene, and clinical standards.
• Participate in ongoing training and skills development.
