Job summary

BrightStar Care – Cleveland is seeking a skilled LPN with Trach & Vent experience to provide high-quality, specialized patient care in home and facility-based settings. Join a supportive clinical team where your airway management, critical care, and complex patient needs make a real impact every day.

This position covers Monday–Friday 8a–4p and weekend morning shifts, with willingness to work in Elyria, Lorain.

Qualifications

• Active Ohio RN license (unencumbered).
• Minimum 1 year clinical experience (home care or critical care preferred).
• Current BLS/CPR certification and TB clearance.
• Ability to lift up to 50 lbs and assist with patient mobility.
• Reliable transportation & valid driver's license.

Responsibilities

• Provide skilled nursing care for pediatric, memory/dementia, and complex care patients.
• Perform and manage Trach & Vent care, including airway and ventilator support.
• Monitor patient condition and respond to clinical changes appropriately.
• Assist with ADLs and patient mobility as needed.
• Implement infection control and patient safety protocols.
• Administer care per physician orders and clinical guidelines.
• Maintain accurate, timely clinical documentation.
• Communicate effectively with care team, patients, and families.
• Ensure compliance with all safety, hygiene, and clinical standards.
• Participate in ongoing training and skills development.
