Job summary

BrightStar Care - Cleveland West is seeking an LPN with Trach & Vent experience to provide high-quality patient care in home and facility-based settings on the Brooklyn night shift. You will join a supportive clinical team focused on airway management, pediatric and complex care, and compassionate, one-on-one patient interactions.

Responsibilities include managing Trach & Vent care, monitoring patient status, assisting with ADLs, medication administration, infection control, and accurate

Qualifications

• LPN with Trach and Vent experience providing care in home and facility settings.
• Experience in pediatric, memory/dementia, and hospital-level care.
• Commitment to compassionate, high-quality nursing care.

Responsibilities

• Provide skilled nursing care for pediatric, memory/dementia, and complex care patients.
• Perform and manage Trach & Vent care, including airway and ventilator support.
• Monitor patient condition and respond to clinical changes.
• Assist with ADLs and patient mobility.
• Implement infection control and patient safety protocols.
• Administer care per physician orders and clinical guidelines.
• Maintain accurate, timely clinical documentation.
• Communicate effectively with care team, patients, and families.
• Ensure compliance with all safety, hygiene, and clinical standards.
• Participate in ongoing training and skills development.
