Job summary

BrightStar Care – Cuyahoga West is seeking an LPN with Trach & Vent experience to provide high-quality care in home and facility-based settings in Elyria, OH. You will support pediatric and complex patients with airway management and compassionate nursing.

You will work Monday–Friday 8a-4p, with weekend morning shifts available; training and RN oversight provided. Join a nationally recognized home care brand and make a meaningful difference in patients' lives.

Qualifications

• LPN with Trach and Vent experience required.
• Experience in pediatric, memory/dementia, and hospital-level care preferred.
• Strong clinical judgment and independent decision-making skills.

Responsibilities

• Provide skilled nursing care for pediatric and complex patients.
• Perform Trach & Vent care, including airway and ventilator support.
• Monitor patient condition and respond to clinical changes.
• Assist with ADLs and patient mobility as needed.
• Implement infection control and patient safety protocols.
• Administer care per physician orders and clinical guidelines.
• Maintain accurate, timely clinical documentation.
• Communicate effectively with care team, patients, and families.
• Ensure compliance with safety and clinical standards.
• Participate in ongoing training and skills development.
