Job summary

BrightStar Care - Cleveland West is seeking an LPN with Trach and Vent experience to provide high-quality, specialized patient care in home and facility-based settings. The role supports airway management, critical care, and complex patient needs on a night shift in Brooklyn, OH.

The ideal candidate holds an Ohio RN license and BLS/CPR certification, with 1 year of clinical experience. Responsibilities include trach/vent care, meds administration, suctioning, ADLs, and comprehensive

Qualifications

• Active Ohio RN license (unencumbered)
• Minimum 1 year clinical experience (home care or critical care preferred)
• BLS/CPR certification and TB clearance
• Vaccinations (COVID-19, Flu, Hep B)
• Reliable transportation & valid driver’s license
• Ability to lift up to 50 lbs and assist with patient mobility
• Participate in ongoing training and skills development

Responsibilities

• Provide skilled nursing care for pediatric, memory/dementia, and complex care patients
• Perform and manage Trach & Vent care, including airway and ventilator support
• Monitor patient condition and respond to clinical changes
• Assist with ADLs and patient mobility as needed
• Implement infection control and patient safety protocols
• Administer care per physician orders and clinical guidelines
• Maintain accurate, timely clinical documentation
• Communicate effectively with care team, patients, and families
• Ensure compliance with safety, hygiene, and clinical standards
• Participate in ongoing training and skills development
