Job summary

Baptist Convalescent Center, Inc. in Kentucky seeks a dedicated LPN to join our night shift nursing team from 6pm to 6am.

The LPN will collaborate with the Nursing Unit Manager and other staff to provide quality, person-centered care and manage medications under physician orders. Requirements include a current Kentucky LPN license and graduation from an accredited nursing program, with experience in long-term care preferred.

Qualifications

• Must possess a current Kentucky LPN license.
• Graduate from an accredited school of nursing.
• Long-term care experience preferred.

Responsibilities

• Provide medication administration as ordered.
• Assist with nursing assessments and care planning.
• Participate in QAPI activities and improvement initiatives.
• Ensure documentation and resident records are accurate and up-to-date.
• Coordinate handoffs and communications between shifts.
• Support orientation and mentorship of new personnel.
