Job summary

Baptist Life Communities in Kentucky is seeking an LPN for night shift (6pm–6am) to join the Nursing Unit team. This role emphasizes person-centered care, medication administration, and collaboration with physicians and staff.

You will participate in QAPI initiatives, maintain accurate clinical records, and contribute to care planning and performance improvement while ensuring compliance with facility policies and state regulations.

Qualifications

• Must possess a current Kentucky LPN license and graduate from an accredited nursing program.
• Demonstrated ability to participate in QAPI activities and apply related guidelines.
• Knowledge of patient care planning and nursing standards required.

Responsibilities

• Administers medications and treatments as ordered by the attending physician per facility policy.
• Maintains resident clinical records per facility policy and nursing standards of practice.
• Develops, implements and revises the Resident care plan as resident status requires.
• Supervises nurse aides and ensures care assignments follow the plan of care.
• Notifies physician and resident/legal representative of changes per policy.
• Supports orientation and acts as liaison between residents, families and staff.
