The Clare, a luxury continuing care retirement community in Chicago, seeks a compassionate Part-Time LPN for the Assisted Living/Memory Care Unit. You will provide nursing care, monitor residents, and support daily activities for residents in a friendly, patient-focused environment.

Night shift with differential, license validation in Illinois, CPR certification required, and a strong commitment to resident well‑being and teamwork are essential.

Qualifications

• Current LPN license valid in Illinois.
• Valid CPR certification.

Responsibilities

• Assist residents with ADL and promote independence.
• Identify medical/behavioral changes and monitor care plans and re‑assess residents.
• Provide daily report to the Director of Health Services regarding conditions and needs.
