ClarkLindsey is seeking a Licensed Practical Nurse (LPN) to provide personalized nursing care to residents in a long-term care/assisted living setting in Urbana, IL. The role involves monitoring health, administering medications, and collaborating with an interdisciplinary team to ensure high-quality care.

Qualifications

• Current and valid Licensed Practical Nurse (LPN) license in Illinois.
• Strong clinical and organizational skills.
• Excellent communication and interpersonal abilities.
• Experience in a long-term care or assisted living setting is preferred but not required.
• Compassionate and empathetic approach to care.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor residents' vital signs and overall health.
• Assist in developing and updating individualized care plans.
• Provide compassionate support and assistance with daily living activities.
• Communicate effectively with residents, families, and other staff such as CNAs, Therapists, Engagement Specialists and leadership.
• Maintain accurate and up-to-date medical records and documentation.
• Respond promptly to residents’ needs and medical emergencies.
• Adhere to ClarkLindsey’s policies, procedures, and regulatory standards.
