Job summary

The Clare in Chicago's Gold Coast cares for residents in a luxury continuing care retirement community. We are seeking a compassionate Part-Time Licensed Practical Nurse to support the Assisted Living/Memory Care Unit during weeknight shifts.

You will assist residents with daily living, monitor changes, follow physician orders, and coordinate with healthcare teams. Illinois LPN license and CPR certification required; night differential applies.

Qualifications

• Active IL LPN license.
• Current CPR certification.
• Night-shift availability for weekends and weekdays on a rotating schedule.

Responsibilities

• Assist residents with Activities of Daily Living (ADL), family/social activities and promote independence.
• Identify medical/behavioral changes and help develop and monitor care plans.
• Provide daily report to Health Services on conditions, needs, and supplies.
• Deliver nursing care per physician orders and standards.
• Coordinate with other disciplines to ensure comprehensive care.
