Wellstar MCG Health Medical Center in Georgia is seeking an LPN for part-time night shifts (24 hours per week, 2/12s) to provide direct patient care in a home health setting. The LPN will work under RN oversight and participate in call rotations and weekend visits as needed.

Responsibilities include coordinating services, educating patients and caregivers, maintaining accurate documentation, and collaborating with the care team to develop and revise patient plans of care.

Competenze

• Minimum one (1) year of medical/surgical nursing practice in an acute care environment is preferred
• LPN license required upon hire where applicable according to state regulation
• BLS certification or equivalent required at hire

Mansioni

• Provide direct patient care to medical/surgical patients under RN supervision
• Coordinate care with RN and other health team members
• Educate patients and caregivers on home health care and treatment plans
• Complete required documentation within established timelines and comply with regulatory guidelines
