Carbondale Rehabilitation and Healthcare Center, a skilled nursing facility in Carbondale, PA, seeks an LPN for 3rd shift (11PM–7AM) with every-other-weekend scheduling. You will deliver skilled, compassionate care and supervise CNAs to ensure high-quality, person-centered services for residents.

The role emphasizes patient advocacy, infection control, and collaboration with RNs and physicians to maintain safety and comfort during night hours.

Qualifications

• Active, unrestricted Pennsylvania LPN license.
• Genuine passion for geriatric and rehabilitative nursing.
• Strong clinical skills with attention to detail.

Responsibilities

• Deliver direct nursing care according to individualized care plans.
• Administer medications and treatments accurately and safely.
• Monitor residents, recognize changes, and communicate with RNs/physicians.
• Maintain clear, timely, and accurate documentation.
• Guide and support CNAs to ensure consistent, excellent care.
• Assist with admissions, transfers, and discharges.
• Build trusting relationships with residents, families, and the care team.
• Uphold infection control and resident safety standards.
