Qualifications

• Current Pennsylvania LPN license in good standing or active multistate license with PA authorization.
• BLS/CPR and First Aid certification required.
• Minimum age 21.

Responsibilities

• Deliver nursing care based on each resident’s individualized care plan.
• Administer medications and treatments per provider orders.
• Monitor residents and promptly report changes to the clinical team.
• Collaborate with physicians, nurses, and other disciplines to coordinate care.
• Maintain accurate nursing documentation and care-plan updates.
• Provide thorough change-of-shift reports to ensure continuity and safety.
• Operate within LPN scope and escalate appropriately.
