Job summary

Apex Home Health, Hospice & Home Care is seeking a Licensed Practical Nurse (LPN) to provide pediatric care, primarily during night shifts. You will assess, plan, and deliver nursing care in the patient's home environment.

The role emphasizes using the nursing process, adjusting care plans with physician directives, administering medications, monitoring patient responses, and coordinating discharge planning while supporting families.

Qualifications

• Current LPN license in the practicing state.
• Current PPD or Chest X-Ray, and BLS certification.
• Preferred: one year of nursing experience.

Responsibilities

• Utilizing the nursing process to plan, implement, and evaluate patient care
• Initiating and adjusting care plans based on patient needs and physician directives
• Administering medications and treatments, monitoring effectiveness, and reporting changes in patient condition
• Incorporating therapeutic, preventive, and rehabilitative nursing actions
• Identifying educational, care coordination, and discharge planning needs
